Copies of pertinent medical records are available to the patient or an authorized representative for a nominal fee once we have received a signed Authorization to Release Healthcare Information Form. The patient may hand-deliver, fax (703) 858-1801 or mail the completed form to our office.

Special documentation requests, other than insurance company requirements, including letters and forms completion, will be honored for a fee.

In accordance with Virginia law, medical records will be destroyed six (6) years after the last documented visit. Records of minors will be kept until they are eighteen years of age.
